GO GROW PLAYROOMS BIRTHDAY PARTY LIABILITY WAIVER & RELEASE

As the host of a private birthday party at Go Grow Playrooms ("Go Grow"), I acknowledge and agree to the following terms on behalf of myself, my child(ren), and all invited guests, including but not limited to children and their parents/guardians:

ASSUMPTION OF RISK

I understand that Go Grow is an indoor play facility designed for young children, and that participation in play activities involves inherent risks, including but not limited to falls, collisions, and other potential injuries. I voluntarily assume all risks associated with participation in play activities, including those that may result from the negligence of Go Grow, its owners, employees, or representatives.

RELEASE OF LIABILITY

In consideration of being allowed to host and participate in a private party at Go Grow, I, on behalf of myself, my child(ren), and all invited guests, hereby release, waive, and discharge Go Grow, its owners, employees, agents, and affiliates from any and all liability for injuries, losses, or damages of any kind arising out of or related to participation in play activities at the facility.

HOST RESPONSIBILITY

As the host of this event, I understand that I am responsible for ensuring that all guests, including children and their parents/guardians, comply with Go Grow’s rules and policies. I acknowledge that:

• All children must be supervised by an adult at all times.

• Shoes must be removed before entering the play area, and socks must be worn.

• Rough play, pushing, climbing on non-play structures, and other unsafe behavior is prohibited.

• Food and drinks are not allowed in the play area.

MEDICAL AUTHORIZATION

In the event of an injury or medical emergency involving myself, my child(ren), or any of my guests, I authorize Go Grow staff to seek medical assistance, including calling emergency services if necessary. I understand that I am responsible for any medical expenses incurred as a result of such incidents.

PHOTO/VIDEO RELEASE

I grant permission for Go Grow to use photos or videos taken during the event for promotional purposes, including but not limited to social media, website content, and marketing materials. If I do not consent, I will notify Go Grow in writing prior to the event.

ACKNOWLEDGMENT & AGREEMENT

I have read and understand this liability waiver and release. I acknowledge that by signing this document, I am waiving certain legal rights. I sign this waiver voluntarily and agree to its terms on behalf of myself, my child(ren), and all guests attending the event.
